"population health." It is a term that is widely used in the world of health care, but not universally understood. Some definitions of population health focus on results. Others focus on the measurement. Still others focus on accountability. So what does the health of the population really mean? Who is responsible? What did he impact on our current health care environment?

In recognition that there is no uniform definition of this important concept and emerge, we sought to create a new dialogue with a variety of opinion leaders in the field. We joined more than 100 health care leaders and asked them to define the term "population health". The following are their answers.

What we have learned

The population health concept got on in 03, when David Kindig and Greg Stoddart defined as "the results of the health of a group of individuals, including the distribution of these results within the group. "Though accurate, some complain of this definition focuses strictly on the measurement of health outcomes, without explaining or recognizing the role that health care providers health must take to influence these results.

Our survey reflects that notion. of the 37 leaders who attended, only two people directly quoted the original definition of Kingdig and Stoddard . Although the interpretation and understanding of the term "population health" differed greatly in the responses we received, many have seen as an opportunity for health care systems, organizations and organizations to work together to improve health outcomes of the communities they serve. Two other key trends and issues that we observed include:

  • Questions of responsibility. Does the health outcomes of a specific population rely on the behavior of the population? supplier's actions? Or both?
  • A take on Triple Aim. Several participants referred the initiative Triple Aim, an approach developed by the Institute for Healthcare Improvement to optimize the performance of the health system.
